Care Coordinator

The nominated care coordinator at our surgery is Rachel Statham.

Care coordinators can help with relationships, housing, mental health and long-term health. For many people in Salford, the cost of living crisis is impacting many people’s health. But help and support is available. For example, did you know your GP care coordinators can provide advice and signpost you to local support?

The care coordinator at the practice can provide extra time, capacity, and expertise to support patients in preparing for or following-up clinical conversations with GPs (primary care clinicians) and the broader primary care teams. 

Our care coordinator follow a Comprehensive Model for Personalised Care to deliver and reflect local priorities, health inequalities at a neighbourhood level and population health management risk stratification.
